Incoming Student-Athletes
Please read this entire page as each section contains imperative information that may impact your participation status.
We are extremely pleased to welcome you to Lynn University for the upcoming academic year. Below you will find important information regarding the pre-participation physical examination process. The following three forms must be completed and returned in to the Athletic Training staff before you will be cleared to participate as a Lynn University student-athlete:
Each new student-athlete (freshman or transfer) is required to have a physical examination by a licensed physician within the United States before participating in intercollegiate athletics at Lynn University. To assist your physician in reviewing your health status, please be thorough when filling out the Medical History forms. It is imperative that all medical procedures, injuries and conditions are noted.
The forms will provide your physician with the necessary information to enable you to be cleared by the University Athletic Training staff. Your physician is to fill out the Physical Form. The Lynn University Athletic Training staff will determine whether a student-athlete needs additional medical clearance from a university team physician in order to participate.
Important: All physicals must be dated after July 1 of the academic year in which you plan to attend Lynn and be complete on the Lynn University Sports Medicine Physical Form. NO OTHER PHYSICAL FORMS WILL BE ACCEPTED. The Athletic Training staff will not accept Lynn University Student Health physical forms. However, Student Health will accept our athletic physical forms--You DO NOT need two physicals.
Please be sure to fill out the Insurance Questionnaire in the event medical attention is required outside the scope of the Athletic Training staff. This information will be provided to the medical vendors for billing purposes. Also, please include a legible copy of the front and back of your current insurance card. Returning Student-Athletes
All returning student-athletes must complete an intercurrent physical examination form, upon returning to campus, in order to be cleared to participate in Lynn University Athletics. The form consists of a brief medical history of the past year, notification of any changes in insurance information including an up-to-date legible copy of the front and back of your insurance card, and offers you the right to see a physician for any reason you deem necessary. Height, weight, vision, heart rate, and blood pressure are also measured by a member of the AT staff. Once this form has been signed and clearance given from a Lynn University AT staff member, the student-athlete will be cleared for participation. However, if determined that further evaluation is necessary, the student-athlete will be referred to the appropriate medical facility for medical clearance.
IMPORTANT: Any student-athlete who is removed from the active roster for any reason (quit, semester off, etc.) and then rejoins the team at a later date must complete the Incoming Student-Athletes protocol above prior to being cleared for participation.
Medical Exemptions Policy (ADHD)
As of August 1, 2009, the NCAA and Lynn University requires all student-athletes who have been diagnosed with ADHD and prescribed banned stimulant medications (Ritalin, Adderall, Concerta, etc.) to submit specific medical documentation. This information is vital to avoid a positive drug test result from Lynn University or the NCAA. Please refer to the Medical Exemptions Policy for the necessary documentation.
Sickle Cell Trait Status
As of August 1, 2010, all student-athletes are required to either show proof of a previous sickle cell trait test or sign a waiver releasing Lynn University from any liability. This must be completed prior to participating in intercollegiate athletics at Lynn University. Those choosing to sign the waiver will do so once arriving on campus. Previous Injuries/Outside Medical Care
Any student-athlete who is under the care of another physician for any recent injuries and/or illnesses, and/or if they have had any orthopedic or recent general medical surgeries, must submit any operative and physician's notes and submit a report and clearance on the physician's letterhead before they will be considered for medical clearance to participate.
Exit Physicals
All student-athletes, both returners and non-returners, are required to complete an exit physical examination, with the assistance of an athletic training staff member, at the end of each year. If determined that further evaluation is necessary, the student-athlete will be referred for the appropriate medical care.
IMPORTANT: Student-athletes who fail to complete the exit exam may be responsible for any additional medical expenses incurred.
Try-outs
Any PSA or enrolled student who participates in a tryout must submit the following:
- Provide a physical examination signed by a licensed physician. The examination must be completed within six months of the tryout and approved by a member of the athletic training staff.
- Sign a Liability Release Form prior to the try-out which is valid only for that day.
- Provide proof of a previous sickle cell trait test or sign a waiver releasing Lynn University from any liability prior to the try-out.
NCAA Interpretation (03/08): A physical examination completed within six months prior to a PSA’s senior year of high school, in which the physical was accepted to clear them for participation in athletics, may be used to satisfy the physical examination requirement.
Likewise, a physical examination completed within six months prior to any two-year or four-year college PSA’s participation at his or her institution, in which the physical was accepted to clear them for participation in athletics, may be used to satisfy the physical examination requirement.
